Southcoast Behavioral Health, located in Dartmouth, Massachusetts, is a premier mental health treatment center with 192 beds. We serve pediatric, adolescent, adult, and geriatric patients, as well as adults with co-occurring mental health and substance use disorders.
PURPOSE STATEMENT:
Manage the client's/patient's/resident’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to clients/patients/residents.
Caseload of 12
Team approach: Work closely with another Case Manager, two attending providers, nurses and BHA’s on the unit to engage the patient in treatment and provide a safe and adequate discharge plan
Completing treatment plans, psychosocial assessments and 2x weekly meetings with each patient.
Collaborate with families and community providers including DMH to coordinate care and discharge plans.
Work Schedule: Monday-Friday 8am-4pm
